Program Guidelines

The patient portion of the bill is due the day of service.
There will be a $50 reinstatement fee if your plan lapses.
It cannot be used in conjunction with another dental plan or financing program.
No refunds of premiums will be issued at any time if participant decides not to utilize the dental plan.

Program Exclusions And Limitations

This program is a discount plan, not a dental insurance plan. It cannot be used

In conjunction with another dental plan, any time-limited specials, dental plan or financing program such as Wells Fargo.
For treatment which, in the sole opinion of our doctors, lies outside the realm of their capability.
For referrals to specialists.
For hospitalization or hospital charges of any kind.
For the cost of dental care which is covered under medical
For services of injuries covered under workers' compensation.

Patients agree that Smiles of Virginia Family Dental Center's fees stated must be paid at the time services are rendered. Any service not paid for at the time of service will be billed at usual and customary fees. Plan fees are valid only when paid at the time of enrollment. All family members must reside in the same household. This is not an insurance product.

Fee's saved is based on utilizing all services that are offered at 100% coverage. You might not need all each year which will be determined after your comprehensive exam. Complete terms and conditions are available at our office.
